How do I qualify for an mmj card in Arizona?
You must have one of the following medical conditions:
• Agitation of Alzheimer’s Disease
• Amyotrophic Lateral Sclerosis
• Cancer
• Crohn’s Disease
• Glaucoma
• Hepatitis C
• HIV/AIDS• PTSD
• Cachexia or Wasting Syndrome
• Severe Nausea
• Severe and Chronic Pain
• Seizures
• Severe and Persistent Muscle Spasms
What do I need to bring in?
Arizona Department of Health requires medical records from a treating physician. This can be documentation of a visit within the last 12 months. If you have seen a doctor in the last year, we can obtain the needed records for you. Download and fill out our medical release form and email or fax it back.
If you have not seen a physician in the last 12 months, you may be able to get an in house examination. Please contact our office to discuss your options.
How much does a medical marijuana card cost?
The doctor’s visit fee is $150 and the fee to Arizona Department of Health (AZDHS) is $150 for the 2 year card or $75 if you have food assistance.
You will meet privately with a board certified and experienced physician. Appointment time is approximately 30 minutes. Unlike most clinics we have little or no wait time.
In addition to getting certified for your medical marijuana card, other pain management treatment options will be made available to you.
After your evaluation
Once your evaluation is completed, an application needs to be processed with Arizona Department of Health at medicalmarijuana.azdhs.gov. The Fee is $150 or $75 with food assistance for the two year card. The card is electronic and you will receive a registration link within 3-5 business days.
